Shawna Yates is a board-certified Family Nurse Practitioner and the founder of Yates Family Healthcare. With a passion for personalized, relationship-based care, Shawna established her practice under the Direct Primary Care (DPC) model to prioritize what truly matters — the patient.

With a background in both primary and critical care, Shawna brings a wealth of clinical experience and a compassionate, down-to-earth approach to medicine. She takes the time to truly listen, educate, and partner with her patients to develop individualized care plans that support lifelong health and wellness.

Shawna is especially committed to making healthcare accessible, affordable, and personal, believing that everyone deserves a provider who knows them and cares about their well-being — not just their chart.

When she’s not in the clinic, Shawna enjoys spending time with her family and exploring the beauty of Southwest Colorado.
